ORDER ONLINE

You will find everything that is represented in our catalog and more! Please allow 7 to 10 business days to receive your order. If you would like Express shipping please place your order by phone with a customer service representative.

Visa, MasterCard, or Discover and approved phone billing customers can order by phone Monday through Friday between 8:00 am to 5:00 pm Mountain time. When placing phone order, please have the information below readily available.

6. Visa, MasterCard, or Discover credit card number, expiration date, and name of credit card holder

ORDER BY FAX
Visa, MasterCard, or Discover and approved purchase orders can be faxed at anytime day or night to 303-794-3704. Please include the type of credit card, the credit card number, the expiration date, the name of credit card holder, daytime phone number with area code (required), and your signature.

ORDER BY MAIL
Clearly print or type the order form completely and mail it with full payment.
1. ORDERED BY: Include first and last name, mailing address and daytime telephone number with area code. UPS will not deliver to post office boxes. If address has changed since last order or differs from the mailing label on catalog, please notify us so we can change our mailing list.
2. SHIP TO: If order is to be sent to an address other than "ORDERED BY" please fill out the name and shipping address section with full street address and zip code. UPS will not deliver to post office boxes.
3. ORDER: Fill in order number, item description, color and/or size, quantity, price of each order number and the total of order. For Colorado residents only, add 7.85% sales tax for all order items except hearing aid batteries. Shipping and handling charges should be added according to the chart on the order form.
4. METHOD OF PAYMENT: Checks, money orders and cashier's checks in U.S. funds only should be made payable to ADCO Hearing Products or ADCO. A fee of $25.00 will be charged on any returned checks. For credit card, please include appropriate type of credit card, the credit card number, the expiration date, the name of credit card holder, and signature.

ORDER BY PURCHASE ORDER
Written or typed purchase orders will be accepted from approved State or Federal agencies, credit approved businesses, and credit approved dealers. Purchase orders written or typed on company letterhead can be substituted. To avoid duplication, do not fax and mail a copy for confirmation unless it is marked as confirming only. Purchase orders can be mailed or faxed and be billed net 30 days.

EXPRESS DELIVERY SERVICE
For an additional charge, we can deliver overnight via UPS Red Label, 2nd day via UPS Blue Label or 3rd day via 3-Day Select Ground. The order must be received by 12:00 pm Mountain Time to guarantee express delivery service.

PROMPT DELIVERY
We will process and ship all in-stock merchandise via UPS within 10 days. Please expect to receive merchandise in 10 to 14 business days from the time we receive order.

SHIPPING ERRORS
Shipping errors must be reported within 5 business days of receipt of order.



BACK ORDERS
Merchandise that is out of stock will be back ordered. We will call or write to notify of back ordered merchandise. At that time, we will notify you when the back ordered merchandise will become available. We will attempt to complete your order in one shipment. Merchandise which is other than back ordered merchandise and is available can be shipped separately per request.

DAMAGED MERCHANDISE
If any merchandise is received that has been damaged in shipping, please do the following:
1. Keep all packaging material in the same condition as it is. Claims can not be made if packing material is missing.
2. Report the damage immediately to the carrier - UPS or Postal Service.
3. Do not ship package back to ADCO Hearing Products; wait for the carrier to inspect and pick up the package.
4. Call Customer Service to report the damage so that we can process a replacement order.

DEFECTIVE MERCHANDISE
When merchandise is defective in some way, please call Customer Service at 800-726-0851 V/TTY or 303-794-3928 V/TTY for instructions on returning it for replacement. Replacement can be made only if the defective merchandise is returned to ADCO Hearing Products within 30 days of the original shipment. Please see Returns and Exchanges below.

RETURNS AND EXCHANGES
Returns are accepted within 30 days of purchase with prior authorization. Before returning any merchandise for credit or exchange, all returns must be pre-authorized and accompanied with a Return Material Authorization (RMA) number. To obtain RMA number, call our Customer Service at 800-726-0851 V/TTY or 303-794-3928 V/TTY. For prompt service, identify the merchandise, the reason for the return or exchange, and have the invoice number readily available. Repack the merchandise carefully in the original carton with all instruction manuals, warranty card, and the original packing materials. Clearly mark the RMA number on the outside of the shipping package. Send the merchandise and packing slip prepaid and insured to: ADCO Hearing Products, 4242 South Broadway, Englewood, Colorado, 80113. Any merchandise returned without the RMA number or unclearly displayed will be delayed in the return process or refused. NOTE: Software, DVDs, Videotapes, and personalized items are not returnable.

WARRANTIES
All merchandise is sold with the manufacturer's warranty and instructions for warranty accompany each product. No other warranty or guarantee expressed or implied is made. Manufacturer's warranties are provided for the customer. Merchandise that fails within the warranty period should be returned directly to the manufacturer with a copy of receipt enclosed. Call our Customer Service to verify the manufacturer's address and/or telephone number. Defective merchandise returned within the first 30 days will be exchanged with the same item. Defective merchandise returned after the first 30 days will fall under the manufacturer's warranty policies.

CATALOG PRICING
All prices are subject to change without notice. Orders will be billed at the prices in effect at the time the order is received. ADCO Hearing Products reserves the right to discontinue merchandise without prior notice and to correct typographical errors. Prices here supersede all previous pricing and are in effect as of the date shown on the order form. Prices and specifications are subject to change.

DISCOUNT POLICY
Discounts on merchandise are available on quantity orders for bookstores, stores, libraries, educational instructions, and resellers. For further information, contact Customer Service.

GIFT CERTIFICATE
Gift certificates are available for any merchandise and in amounts of $15.00 or more. We can send gift certificates to the designated name and address. Please do not add shipping charges to the gift certificate.


ARE YOU MOVING?
If you are moving, please let us know your new address so you will not miss new catalogs. We need both your old and new addresses for verification. Please also indicate whether it is an individual or household move.

ARE YOU RECEIVING DUPLICATE CATALOGS?
If you are receiving duplicate catalogs, please send us all the mailing labels and indicate which one you prefer. We will correct our mailing list at once. Please share the extra catalog with your friend.
